1. Information we collect

• Information you provide. When you submit our Contact form or Membership Application, we collect the information you enter, such as your name, title, practice name and type, practice address, phone number, email address, estimated annual vaccine volume, and the content of your message.
• Information collected automatically. Like most websites, we use Google Analytics to understand how the Site is used. This collects information such as pages visited, approximate location derived from IP address, device and browser type, and referring website. We do not use this information to identify you personally.
• Information we do not collect. The Site does not collect patient information, payment card details, or Social Security numbers. Please do not include protected health information in form submissions.

2. How we use information

We use the information you provide to respond to inquiries, review membership applications, set up and administer memberships, communicate with you about VAXgpo programs and manufacturer contracts, and improve the Site. We may contact you by email or phone using the details you provide.

3. How we share information

We do not sell personal information. We share it only with: service providers that help us operate the Site and process form submissions (for example, our website host and email delivery provider), who may use it only on our behalf; vaccine manufacturers, distributors and other partners as needed to enroll your practice in contracts you have requested; professional advisers; and authorities when required by law or to protect our rights.

4. Cookies and analytics

The Site uses Google Analytics, which sets cookies to measure Site usage. You can opt out with the Google Analytics opt-out browser add-on (https://tools.google.com/dlpage/gaoptout) or by disabling cookies in your browser. The Site does not display advertising and does not use advertising cookies.

We keep form submissions for as long as needed to respond to you and administer your membership, and as required by law. We use reasonable administrative and technical safeguards to protect information, but no method of transmission over the internet is completely secure.

6. Your choices

You may request access to, correction of, or deletion of the personal information you submitted through the Site by emailing info@vaxgpo.com. You may unsubscribe from marketing emails using the link in any such email.

7. Links to other sites

The Site links to third-party websites (for example, the CDC, FDA and medical societies). We are not responsible for their content or privacy practices.
